We are seeking a calm, reliable support worker to assist a young female with multiple health needs. She attends regular hospital appointments and can become overwhelmed by changes to routine, particularly during periods of low mood. 
 
The role involves supporting with planning and arranging appointments, booking patient transport, providing reassurance that plans are in place, attending appointments when required, and helping with letters and forms.

Consistency is essential, as having the same support worker helps reduce anxiety and allows flexibility for short-notice appointments. 
 
The young female is wheelchair dependent when leaving the house and relies on public and private transport. 
 
Hours: 4 hours per week (flexible)
Rate: £13.74 per hour 
 
Experience in a support or care role is desirable but not essential.

A calm, organised, and reassuring approach is key.  
 
Enhanced DBS and references required.
