Job Purpose:
I am an elderly gentleman with a diagnosis of Alzheimer's and dementia who is looking for a Personal Assistant to support me with day-to-day tasks. I value my independence and prefer to handle the tasks myself but may need your support with completing them. This would include personal care tasks like bathing, changing clothes, and toileting support. You will help prompt and encourage me on the importance of performing these tasks regularly.

In addition to personal care, you will support me with meal preparation and planning to ensure I have balanced and nutritious meals. This may involve coordinating with meal delivery services that are ordered by the family and delivered every Tuesday.

I require support with medication management. You will help to ensure I take my medication as prescribed, keeping track of schedules and refills to manage my health conditions effectively.
You will also assist with my mobility around the home and when out and about. I enjoy exploring my local area, however, due to my declining mobility, I require assistance to go out when necessary. I like to visit local parks which provide me with a change of scenery and help reduce feelings of loneliness.
During visits you will perform domestic chores as needed, including laundry and housekeeping tasks. This involves helping me with washing clothes and maintaining a clean and safe living environment. 
You will need to coordinate with my family, involving regular communication to update them on my condition and any additional needs. You will support me in maintaining my autonomy by encouraging me to participate in activities I enjoy and can manage safely. This will help with my desire for independence and enhance my quality of life.

Summary terms and conditions:
· The rate of pay for this position is £12.38 per hour.
· Hours and times of work: 6 hours per week with a schedule of 2 hours on Monday, 2 hours on Wednesday, and 2 hours on Friday, exact timing will be advised ahead of time.
· The annual holiday entitlement for this position will be equivalent to the work I would normally expect to be done during 5.6 consecutive weeks.
· This position requires the employee to complete the following free annual training: GDPR, Health and Safety, Food Hygiene, COSHH, Manual Handling of Objects, Medication Practice and Personal Care. 

Post is subject to the following:
· A satisfactory enhanced Disclosure & Barring Service (DBS) check, the cost of which will be covered by the employer.
· Satisfactory references from two referees, one of which must be your current or last employer.
· Completion of a satisfactory probationary period of 6 months.
