“My name is Nathan,  I live at home with my Mum and Dad.  I use a wheelchair and need someone to help me.
 
I love being out in the community and will need someone to push my wheelchair to get me places.  I love being out, going for walks, meeting people and watching the world go by.  I also like watching films.  You could help me to do my physiotherapy exercises and learn about intensive interaction.  I will also need support with personal care. “
 

Purpose of Job
          
To be a good companion, who has an understanding of what is required for the role of a PA.  To encourage independence, wherever possible.  To be flexible, both in attitude and with the working day. You will be expected to keep total confidentiality at all times. It is hoped that you will have the ability to use your initiative while you are working with me. 

I attend a day centre twice a week, my PA will support me at the centre and encourage me to take part in activities.
 
You will be encouraged to undertake any training courses that will enhance your role as a PA, such as moving and handling/hoist training.  You must be fit and healthy enough to support me by pushing my wheelchair and hoisting me.  You must undergo an enhanced DBS (formerly CRB) check.  
 
You will be entitled to 5.6 days paid holiday each year, pro rata.  You will be required to give advance notice for any holiday required so that alternative arrangements can be made. 
 
Tasks
 
 
·       Ensuring that we get to whatever activity is planned on time and safely
 
·       Ensuring that I am stimulated and happy
 
·       Support me to maintain and develop my methods of communication, using objects of reference and pictures, building on the methods used by my family.
 
·       Support me to access intensive interaction by learning the techniques used by my family.
 
·       Support me to access the activities that I enjoy, i.e. going for walks, going to the cinema, going out in my local community, reading to me
 
·       Accompany my family on day trips when required
 
·       Supporting me to research new activities that I may want to try
 
·       To provide support with dressing and undressing
 
·       To provide support with all personal care
 
·       To provide support with bathing
 
·       Help relieve my stomach cramps when these occur
 
·       Support and encouragement to do my physiotherapy exercises
 
·       To ensure that my posture does not cause me to choke 
 
·       To ensure that you follow the risk assessment regarding my epilepsy
 
·       To ensure my safety at all times as I have no awareness of danger
 
·       Any other request that my parents may reasonably make

Days required
Week 1 -  Tuesday  Wednesday, Friday 
Week 2 - Monday , Wednesday,thursday, Friday 
 
My PA's may be requested to work overtime to cover other team members.
